Free in a Sephora or Sephora at Kohl's store; on sephora.com it needs a $25 order.
How: Join Beauty Insider (free) and save your birthday. In a Sephora or Sephora at Kohl's store, give a Beauty Advisor your Beauty Insider email: no purchase. On sephora.com it needs a $25 merchandise order (gift cards don't count); on Kohls.com there's no minimum.
